If you have NOT uploaded your registration documents prior to the assessment, please bring .....
- Child's birth certificate or Passport
- Current copy of your child's Immunization Records
- Proof of Residency - Please visit our Kindergarten Page on our website for the items requested for this
- Physical and dental forms (if they are complete)
- Completed Home Language Survey
- If there is a custody agreement pertaining to your child, please be sure to bring it with you.
And, of course, don't forget to bring your child!
