The following REQUIRED documents MUST be presented to the Upper Level of District Central Office to complete the enrollment process for all
New Students and Re-Enrollments.

Enrollment Office Hours:
8:00 AM - 3:30 PM
• Birth Certificate
• Valid PA Driver's License or Valid PA Photo Identification Card
• Proof of Parent/Guardian Residency with YOUR name and address.
One (1) document from each category is required.
CATEGORY A - one (1) document:
Current Lease / Rental Agreement
**Lease must be signed by parent/guardian and landlord and show dates
Current Mortgage Statement / Home Purchase Contract / Deed CATEGORY B - one (1) document:
**Mortgage documents must show parent/guardian name(s) and address.
Current utility bill (electric, gas, or water bills only.)
• Custody Papers (if applicable)
• From Student's previous school (if applicable)
Transcript (Grades 9-12 only)
