Registration forms can be downloaded from our website or picked up at our District Administration Office located at 1300 Fifth Street.  All completed registration forms must be returned to our Central Enrollment Office at the District Administration Office.
 
The following documents must be presented at time of enrollment:
(Acceptable documents to establish residency include a deed, a lease, utility bills, tax receipt, vehicle registration or driver's license. The District may require that more than one form of residency confirmation be provided.)  If the utility bill, lease, or sales agreement is not in your name, you must complete a Certification of Residency form and have it notarized.

MASS NOTIFICATION SYSTEM: 

Parents can receive messages via home phone, cell phone, e-mail and/or text message.  Messages will include emergency information, school delays/cancellations, school events, etc.  Please click on the link below to complete this form.  (Only one form needs to be completed per family.)   Mass Notification Form

If you have moved to a new address, please complete this form and return it with proof of residency to the District Administration Office located at 1300 Fifth Street.  Forms can also be mailed, faxed or e-mailed per instructions on the form.
